what's the difference? i.e. my cooking skill is 21.5, my real cooking skill is 11.4... what does it mean? should i care about real or normal skills?

You should only care about Real Skills. Thats the only thing I pay attention to anyway.

Take healing and cure for example. To cure you need 60 healing and 60 anatomy or something like that. Your real skill is 59 healing but your value is 61... This will let you cure even though your real skill is only 59.

A skill 'Value' takes into consideration your stats. This is negated at 100.0 skill, but below that there is a bonus added to your skill depending on what stat modifiers it has.
For instance, if two people have 10.0 base meditation, but one player has more intelligence, then that player will also receive a higher skill value.
A lot of skill calculations are actually checked against your value, and not your base. Crafting boats for instance requires 75 Carpentry. This is 75 Value, not 75 base. If you have high enough stats you can hit this 75 value with as little as 70.0 base.
